About Medserv
Medserv delivers healthcare administration and billing services to healthcare providers across Ireland and the UK. Our software supports daily medical practice operations, including billing, claims, payments, appointments, and patient administration.
The Role
This is Medserv's most senior hands-on individual contributor engineering position. You will lead the technical modernization of our platform, which supports medical billing and practice management services. While remaining substantially hands-on, you will also help shape architecture, technical standards, and guide the transition from legacy systems. Your initial focus will involve defining the direction for our modern platform, strengthening engineering practices, and delivering new capabilities in claims, medical coding, billing automation, and practice management. You will significantly influence technical decisions across the platform, dedicating most of your time to designing and building software.
What You'll Do
- Design and build reliable, secure, and maintainable production software.
- Take ownership of complex technical problems from discovery through to production.
- Shape architecture, APIs, data models, and service boundaries.
- Modernize existing systems while ensuring support for critical business operations.
- Write high-quality production code and review critical implementations.
- Improve automated testing, reliability, security, and observability.
- Mentor engineers and help establish consistent engineering standards.
- Collaborate closely with Product and Engineering colleagues to translate business problems into practical technical solutions.
Our Technology Stack
PHP, Laravel, MySQL, React, TypeScript, AWS, Redis, GitHub Actions, Sentry, and Playwright.
Essential Experience
- Senior-level technical experience, including ownership of complex production systems and influencing technical decisions beyond individual delivery.
- Strong commercial PHP experience, ideally within a Laravel environment.
- Proficient knowledge of relational database design and MySQL.
- Extensive experience designing production systems and APIs.
- Experience mentoring engineers or providing hands-on technical leadership.
Desirable Experience
- Frontend experience with React, Vue, or TypeScript.
- Modernizing legacy systems while maintaining live services.
- Experience in healthcare, billing, payments, or other regulated/operationally complex environments.
- Familiarity with observability tools such as Sentry, Datadog, or Grafana.
- Experience with AI or LLM-assisted product capabilities in production.
- Building and operating production systems in AWS, Azure, or other cloud platforms.
Prior experience with a "Staff Engineer" title is not required; we value the level of operation, problem-solving, and technical influence demonstrated. We also encourage applications even if you don't meet every desirable criterion, provided essential requirements are met.

Additional Information
This is a remote role based in the UK and/or the Republic of Ireland. Occasional attendance at our Dublin office is expected for key planning sessions, design workshops, and collaborative activities.
